Inconsistent 'skip nav' placement

/guides seems to have the correct tab order. After you get through the header you go to the menu nav on the left and the 'skip nav' option appears.

This is not implemented consistently throughout the guide which is an accessibility issue as it's confusing if you expect a page to behave the way the previous page did.

Two examples:

1. /accessibility the tab order jumps to the content before the side menu nav.
2. /agile the 'skip nav' appears before everything, even before the header, and then tab order again goes to content before the side menu nav.
