[FEATURE]: Reporting method step (№4)

Description

As a CC3 adopter, I should be able to define my method of reporting violations to my code of conduct, so I can clearly communicate to end-users how to file a report.

Proposed solution

This step should contain a text field asking users to input reporting method details or instructions. This is important and is highlighted as a separate step to enforcement, which is a following step. (#32)

Help text with example reporting methods should be provided to assist users.
