Alternative team_no assignment strategy

Instead of assigning team indices at registration time, shift them to just before schedule generation. Not only does this make registration faster, but also avoids the truckload of errors otherwise associated with on the fly assignment. (Duplication, and all the issues that follow.)

Invalidated by the realization of #43.
