It seems to occur when placing a Universal Spawn Point on the Mun. I have replicated this on a minimal modded install and a new sandbox game:
I have tested this by placing a Squad Mobile Landing Pad and a Universal Spawn Point each on Kerbin, Mun, Duna, and Ike. The issue only occurs on the Mun when the USP is configured as a launchsite. A USP on the Mun alone doesn't cause the issue.
Create a new group on the Mun, add the USP, and give it a launchsite. At this stage, the site works as expected (model visible, can launch on it from VAB)
Save, close the game, and reopen it. The game will refuse to load.
The game will go past the initial loading screens but freeze before the main menu appears, when the lower-right loading icon appears. This will persist for several minutes until the game closes itself. This is only fixed by removing the launchsite code from the config file.
