Reproduction:

Use the fun tab as admin to make small explosions on /turf/open/floor/wood/f13 and certain vault floor types.
It results in erroneous icon_states, as per a damage proc in code\game\turfs\open\floor.dm

I will add, I don't think a good solution to this is to add a million new damaged floor icons for all icons in our repo.
