I have two profiles configured in Nheko, one for an account with my real name and one for my pseudonymous alt. I choose between them by invoking Nheko as nheko --profile default or nheko --profile sparks respectively. Only the first of these is able to receive desktop push notifications.
Admittedly I haven't tried creating a third account to rule out that it's some random thing I've configured somehow, but I've compared the settings dialogs of the two profiles side by side and they're identical.
To Reproduce
Create two Matrix accounts
Set them up as separate profiles in Nheko, one as the default profile and one as another named profile
Open them simultaneously in separate instances of Nheko
Send a message to the default account from the non-default account -- observe that a desktop notification appears
Alt+tab to the other Nheko window and send a message to the non-default account from the default account -- observe that it does not
