Simplify the Teach Behavior input as a dynamic form with one dropdown question #3770

The request is to have a simple form that drives the behavior (or attendance or scoring) forms in the student lists in Teach.

[ ] Logic in Tangerine platform to have a "magic form" called "behavior/form.html" that has one question.

[ ] Update tangy-form with a control in the tangy-form settings that enables a simplified one-question form

This form should hide the Submit button and fire off the saving logic when the selection is made. The view should be small to fit into a table.

To Discuss: Which form types can we allow here?
-- Start with the simple form types: tangy-input, tangy-select

Bonus: Include a new tangy-widget that allows image selection -- similar to the stop-light used currently in attendance
