If the mandatory filed is blank , Issue Templates will be appended again when creating a issue #76

Hi

If the mandatory filed is blank when creating a issue ,
everytime when clicking "create" botton , Issue Templates will be appended again.

Same Issue when i change the tracker type. The new default template is added at the bottom.
Any chance to get this fixed?
