Clean up index when packages go away. #6

There are a couple of cases when a package goes away:

The source repo is deleted.
The package is deleted in the barbarian branch.
The barbarian branch is deleted.
Others?

Add handling of such circumstances to the index processing to remove, or mark, such packages in the index.
