Once a patient has successfully received charity care, the ACT and referrals teams will notify the Head TS of the week, confirm that the referral is still necessary, and schedule the patient's appointment.

<input type="checkbox" name="checkbox-2z" id="checkbox-2z" class="custom" data-mini="true" /><label for="checkbox-2z"> Give your patient the 19.81 voucher (found on Referrals google site). Voucher can only be used if they attend first FS appointment.</label>
<input type="checkbox" name="checkbox-2b" id="checkbox-2b" class="custom" data-mini="true" /><label for="checkbox-2b"> Find mutually convenient time M-F 8-5 that week for you to go to FS with your patient.</label>
<input type="checkbox" name="checkbox-2c" id="checkbox-2c" class="custom" data-mini="true" /><label for="checkbox-2c"> Ask your patient to meet you at the front desk on the ground floor of CAM (the building EHHOP is in).</label>
<input type="checkbox" name="checkbox-2d" id="checkbox-2d" class="custom" data-mini="true" /><label for="checkbox-2d"> Update the problem list with the date you will go to FS with your patient.</label>

<p> Once your patient has charity care, please contact the <a href="mailto:[email protected];[email protected]"> Emerging Referrals Manager.</a> The Emerging referrals manager will give you directions on how to schedule your patient for their specialty referral appointment.

<p> <b>Cost to EHHOP Patient:</b> Once your patient submits documents confirming their
income, Financial Services will grant you patient Charity Care and place them on a
sliding scale. Most EHHOP patients are placed on Level 1. For patients with Level 1,
the Pulmonology appointment will cost <b>$15</b>. To find out the costs for patients
who are not Level 1, please contact the Emerging Referrals Manager, Kristen Ng
</p>
<p><b>Cost to Society:</b> Outpatient pulmonology appointment cost ~300 in new york
out of pocket.

<div data-role="collapsible"><h3><b>Wait time</b></h3><p> How long it takes patients to complete their appointment will depend on how soon the patients can submit all their documents confirming their income to Financial Services (on Average this takes <b>20 days</b>). Once all documents are submitted, it takes about <b>3 weeks</b> after a patient has received charity care for a pulmonology appointment to be scheduled.</p> </div>
