Remove missing LHS and endog observations from sample

To fix a previous sample marking bug, I had to change mark to only take in the instrument. This will overlook observations with missing LHS or endogenous variables. One possibly dirty way to handle this is just to create a new variable that is the superset of the LHS and endog variables.
