Patient is assigned multiple working group while assigning clinician to the patient

Description:
A Patient can be assigned to one working group only. But it is found that a patient get access to multiple working group when a clinician with multiple working group is assigned to the patient.

Steps to follow:

1. Register a new user (as a parent) and add a new patient
2. Activate the user through email activation.
3. Login as parent(username/password)
4. Accept the consent form and save
5. Logout as parent

1. Login as admin
2. Go to User - select Clinical User and assign multiple working group
3. Logout as admin user

1. Login as clinical staff (clinical/clinical)
2. Go go Menu and click on Patient List
3. Select the registry and select the patient from the list
4. From the left menu - click "Clinician"
5. As Preferred Clinician select one that was assigned multiple registry
6. Click Save
7. Go to Demographics of the patient and I can see multiple working groups are assigned to the patient
8. Click Save and get Validation error

Expected Result:
Assigning a clinician to a patient should not assign multiple working group to the patient.
