Add explanation to reference "the centre"

In the text for the notes from the NHS-R vision from 2021 "the centre" is mentioned. This needs to be clarified as being NHS England as it is an term often used by NHSE to refer to itself and is not commonly known.

As these were notes this clarification should be in an aside format.
