What could result in a naked anonymous testcase being written to the xml?

We're seeing some curious empty testcase elements show up a few of our the junit xml files, they seem to correlate with instances of internal pytest errors. However with pytest internal testcase instances not every instance has the bare unnamed testcase. So it is the case that across the history of our junit xml files we have some with pytest internal test cases but no bare unnamed test case present. Therefore they don't always occur together necessarily.

Would anyone know what part of the pytest codebase could result in these unnamed testcase elements?
