Lazywarden in Unraid

After many frustrating hours I may have finally broken through. Some of the issues that I am having:

I had to fill out all of the variables. I was under the impression that I only needed a minimum of five.
I am presented with a prompt asking to me use the arrow keys to choose either Authenticator App or Email when in the log window.

Using the arrow keys does not work

I would also add that the Unraid template is configured as variables only and not as paths. This wasn't frustrating at all because of the way the template was done to realize.
