New default ETL tasks: allergyintolerance, device, diagnosticreport, and immunization

I just released support in the ETL for four new resources/tasks! 🎉

• AllergyIntolerance (task allergyintolerance)
• Device (task device)
• DiagnosticReport (task diagnosticreport)
• Immunization (task immunization)

These are run by default (but are basically no-ops if the source data isn't available).

We will probably use these in a future quality-metric study. But no firm immediate plans to integrate these into the Cumulus Library. We are just rounding out the USCDI list of resources with an eye towards a metric study.

If you are interested in using these yourself, here's how to get started:

• Update to the latest and greatest Cumulus ETL docker image
• Make sure you can get these new resources from your EHR
• Run Cumulus ETL on those four resources
  • If you usually export separately yourself, export those resources first and provide them to the ETL
  • Otherwise, provide bulk export authorization to ETL and it can export them for you
• Update your CloudFormation template to include the new tables (see example here)
• Run the AWS Glue Crawler to let Athena see the new ETL tables
• Now you should be able to see & query the new tables in AWS Athena!
