Contact Forms

The contact forms (design and access) are managed by BIT.

Before Requesting a Contact Form

The contact forms are hosted on redcap.med.upenn.edu which is managed by PMACS.

You will need a PMACS account and REDCap access before requesting a form.

If you do not have those accounts request in #it_issues.

Requesting a Contact Form

Post in your study channel that you are requesting a contact form.

Include your IRB number, who will need access, and who will get notifications for submissions (all users must have PMACS/redcap.med accounts before requesting).
