Remove Feedback Form from Help & Support

As noted at pfln9p-1ns-p2#comment-1554 we're going to remove the Feedback Form from the Help & Support screen and rely on it for feature-specific feedback, such as the block editor.

The reason for this change is that despite getting a few morsels of good feedback from this feature, we're getting far more junk feedback and unanswerable support requests.
