Add environment variable to disable/enable keys

Purpose

Fixes #1617

This PR adds an environment variable disable key access for OpenAI, defaulting to true.
Developers can set to false if they need key access for some reason.

Does this introduce a breaking change?

When developers merge from main and run the server, azd up, or azd deploy, will this produce an error?
If you're not sure, try it out on an old environment.

I also added some missing docs here - we have this in the deploy_features doc, but didnt have it in this one.

Here's the doc relevant to this change. I tried to make it clear that they shouldnt need to run the command unless really needed.
